How to Train for the Annapurna Base Camp Trek
Train the part that actually ends trips
Almost everyone trains the wrong half of this trek. You’ll train the climb — the cardio, the hills, the step machine going up. Then we put a watch on the route, and the injuries were all on the way down: a 2,086 m descent your heart rate reads as easy, and stone steps back to Chhomrong where 80–90% of trekkers cramp. Lung fitness gets you up. It does nothing for the down. Here’s the measured evidence for why the descent is the crux
Training your cardio for the altitude doesn't prepare you for everything.
Cardio helps you climb, but the injuries come on the way down, on long descents with tired legs — and lung fitness does nothing for that. What protects you is descent-specific strength, and it’s exactly what most training plans skip.
How to train — for the descent, not just the climb
Kak Yaa’s prescription, plus what she’s noticed in the people who don’t cramp:
- Train with load — 8 to 11 kg on your back, not an empty daypack.
- Train stairs in both directions. She weights it 70/30 up. We’d put more into the down.
- Poles are mandatory, and train with them. Buy rather than hire — a usable pair in Pokhara starts around RM 30 and runs to about RM 300.
- Set your poles longer for descending — most first-timers set them once and never touch them again.
- Knee history? Knee brace, plus knee-specific work before you fly.
- Strength training and stretching. Asked what the 10–20% who don’t cramp have in common, this was her answer — the best-supported prevention advice we have.
Where even the experts train it backwards
We asked Kak Yaa how she prepares people. Her answer, quoted with her agreement:
Her focus is the summit. The descent is secondary. Her job is to make sure people summit.
So she splits training roughly 70% ascent, 30% descent. Her logic: if an ankle goes on the way down there’s a horse or a helicopter; if you can’t get up, the trip has failed. That’s not negligence — it’s what the whole industry optimises for, because a trip is judged on the summit. She’s just the only one who says it out loud.
Put more into the descent than 30%
Given where the injuries actually happen, we’d weight training toward the way down, not away from it — that’s us arguing with our own source. The Trek-Ready Program is built on the reversed weighting.

What to do when you cramp
Not medical advice — this is what Kak Yaa does on the trail, and she has done it many times:
- Get them sitting on a flat surface.
- Take the laces out to free the shoe.
- Have them point at exactly what’s cramping.
- Calf → dorsiflexion, pulling the toes back toward the shin.
- Quads and adductors → analgesic spray.
- Rest five to ten minutes, taking electrolytes.
- Stand up and stretch again.
- Walk on with smaller steps.
Her only operational change for that day is to start early.

The hard part is turning it into twelve weeks of the right work, in the right order, from wherever your body is now — the descent conditioning, the cold habituation for freezing teahouse nights, the altitude readiness, and the unglamorous aerobic base that carries the day-after-day load.
